Oświadczam, że dane i informacje zawarte we wniosku są prawdziwe oraz, że jestem świadomy, że złożenie wniosku
lub dołączenie dokumentów zawierających nieprawdziwe dane osobowe lub fałszywe informacje, a także zeznanie w
postępowaniu o udzielenie zezwolenia na pobyt stały nieprawdy, zatajenie prawdy, podrobienie, przerobienie
dokumentu w celu użycia za autentyczny lub używanie takiego dokumentu jako autentycznego spowoduje odmowę
udzielenia zezwolenia lub jego cofnięcie.
Oświadczam, że znana jest mi treść art. 233 § 1 i 6 Kodeksu karnego zgodnie z którym kto, składając zeznanie mające
służyć za dowód w postępowaniu sądowym lub w innym postępowaniu prowadzonym na podstawie ustawy, zeznaje
nieprawdę lub zataja prawdę, podlega karze pozbawienia wolności do lat 3. Powyższy przepis stosuje się odpowiednio
do osoby, która składa fałszywe oświadczenie, jeżeli przepis ustawy przewiduje możliwość odebrania oświadczenia pod
rygorem odpowiedzialności karnej.


I hereby declare that the data and information included in the application are true and that I am aware that the submission of the
application or attachment of documents containing untrue personal data or false information as well as stating the untrue,
concealing the truth, forging, processing document for the purpose of using as authentic one or using such a document as an
authentic one in the proceedings for granting permanent residence permit shall result in the refusal or cancellation of the permit.
I hereby declare that I am familiar with the contents of Article 233(1) and (6) of the Penal Code, which stipulates that whoever, in
giving testimony which is to serve as evidence in court proceedings or other proceedings conducted on the basis of an Act, gives false
testimony or conceals the truth shall be subject to the penalty of deprivation of liberty for up to 3 years. This provision shall be
applied accordingly to a person providing a false statement if a provision of an Act provides for the possibility of obtaining a
statement under the threat of penal liability.

Je certifie que toutes les données et informations contenues dans la présente demande sont véridiques et que je déclare être conscient
que le dépôt d’une demande ou des documents en annexe contenant des coordonnées ou informations fausses, ainsi qu’une fausse
déclaration lors de la procédure de la délivrance du permis de residence permanente, la dissimulation des faits, falsification ou
rectification d’un document dans le but de l’utiliser comme authentique ou l’utilisation d’un tel document comme authentique
causera le refus ou l’annulation de l’autorisation.
Je certifie que je connais le contenu de l’art. 233 § 1 et 6 du Code pénal, selon lequel toute personne qui, lors d’une déposition
destinée à être utilisée comme preuve dans le cadre d’une procédure judiciaire ou autre procédure menée conformément à la loi, nie
les faits ou dissimule la vérité, encourt une peine d’emprisonnement allant à trois ans. Cette disposition s’applique à la personne qui
fait une fausse déclaration, si la loi prévoit la possibilité de recevoir une déposition soumise à l’obligation de sincérité sous peine de
poursuites pénales.

POUCZENIE / INSTRUCTION / INSTRUCTION


1) Należy wypełnić wszystkie wymagane rubryki. W przypadku braku niezbędnych danych wniosek
może być pozostawiony bez rozpatrzenia.
All required fields should be filled in. If necessary data are missed the application may be left unexamined.
Il faut remplir toutes les cases demandées. En cas d’absence des données nécessaires, la demande pourrait ne
pas être instruite.

2) Formularz należy wypełnić czytelnie, drukowanymi literami wpisywanymi w odpowiednie kratki.
The application should be filled in eligibly, in capital letters entered into the appropriate boxes.
Le formulaire doit être remplie lisiblement, en majuscules inscrites dans les cases correspondantes.

3) Części A, B, C, D i G wypełnia wnioskodawca.
Parts A, B, C, D and G should be filled in by the applicant.
Les parties A, B, C, D et G sont à remplir par le demandeur.

4) Część E i F wypełnia organ rozpatrujący sprawę.
Part E and F should be filled in by the authority examining the case.
La partie E et F est à remplir par l’organe qui instruit le dossier.

5) W części A, w rubryce „płeć” wpisać M – dla mężczyzny, K – dla kobiety; w rubryce: „stan
cywilny” należy użyć sformułowań: panna, kawaler, mężatka, żonaty, rozwiedziona, rozwiedziony,
wdowa, wdowiec.
In part A in the „sex” field „M” should be entered for a male and „K” for women; in „marital status” fields
the following terms should be used: unmarried, married, divorced, widow, widower.
Dans la partie A, rubrique „sexe” – inscrire: „M” pour un homme, „K” pour une femme; rubrique „état
civil”, utilisez une des formulations suivantes: célibataire, marié, mariée, divorcée, divorcé, veuve, veuf.

6) W części G podpis nie może wychodzić poza ramki.
In Part G the signature may not exceed the box.
Dans la partie G, la signature ne doit pas dépasser l’encadrement.

7) Części D, V nie wypełnia się w przypadku małoletniego cudzoziemca urodzonego na terytorium
Rzeczypospolitej Polskiej, cudzoziemca poniżej 16 roku życia oraz cudzoziemca, który uzyskał azyl
w Rzeczypospolitej Polskiej
lub będącego ofiarą handlu ludźmi w rozumieniu
art. 115 § 22 Kodeksu karnego, o którym mowa w art. 196 ust. 1 pkt 5 ustawy o
cudzoziemcach.
Part D, V is not filled in for a minor foreigner born on the territory of the Republic of Poland, a foreigner
below 16 years of age and a foreigner, who obtained asylum in the Republic of Poland or who is a victim of
human trafficking within the meaning of Art. 115 § 22 of the Criminal Code referred to in Art.
Paragraph 196. 1 point 5 of the Foreigners Act.
Les Parties D, V ne sont pas remplies dans le cas d’un étranger mineur né sur le territoire de la République de
Pologne, d’un étranger de moins de 16 ans et d’un étranger qui a obtenu le statut de réfugié en République de
Pologne ou qui est victime de la traite des êtres humains au sens de l'art. 115 § 22 du Code pénal visé à
l'art. Paragraphe 196. 1 point 5 de la loi sur les étrangers.
